Case 2 - AFP Negative Liver Mass
Initial presentation
A 68 year old male presents with back and abdominal pain. Further investigation
reveals a 6 cm liver mass and retroperitoneal adenopathy.
Work up:

Initial treatment:
- Carboplatin and Taxol® x 6 cycles with mass at 5.5 cm; symptoms improved
- upon treatment
- During 4 month treatment holiday, the mass grew to 8 cm and pain increased
- New treatment of Taxotere® x 2 cycles; the mass grew to 9 cm and the patient
- complained of more pain
Pathwork® Tissue of Origin Test ordered.
Results - Tissue of Origin Test:
Interpretation from Pathwork Diagnostics’ pathologist: “The Tissue of Origin Test, run on tumor tissue from this 68 year-old man's liver mass, shows that the highest Similarity Score is 87.6 for Hepatocellular. There are 14 other tissues with Similarity Scores less than or equal to 5. The Tissue of Origin Test suggests a Hepatocellular origin in this case with HIGH PROBABILITY. The Tissue of Origin Test has effectively ruled out a derivation from the 14 tissue types with scores ranging from 0.1 to 3.7, as Similarity Scores below 5 indicate a greater than 99% probability of that tissue type not being the correct tissue of origin."

Post Tissue of Origin treatment:
- Therapy changed to sorafenib
