Case 1 - Axillary Mass: Breast vs. Lung Cancer
Initial presentation:
A 41 year old asymptomatic, pre-menopausal, non-smoking female who presents with a 3 cm left axillary mass and no breast abnormalities.
Work up:

Initial treatment:
Carboplatin and Taxol®
Pathwork® Tissue of Origin Test ordered.
“Chemotherapy needs to cover both breast and lung.”
Results - Tissue of Origin Test:
Interpretation from Pathwork Diagnostics’ pathologist: “High probability of breast cancer. The Tissue of Origin Test has effectively ruled out a derivation from the 12 tissue types with scores ranging from 4.9 to 0.2, as Similarity Scores below 5 indicate a greater than 99% probability of that tissue type not being the correct tissue of origin. Specifically, Non-small Cell Lung, with a Similarity Score of 3.0, is excluded by the Tissue of Origin Test.”

Post Tissue of Origin Test treatment:
- Chemoprevention with tamoxifen indicated to prevent contra-lateral breast cancer
- Carboplatin and Taxol changed to dose dense AC/Taxol
- Testing for familial predisposition to breast and ovarian cancer indicated (BRCA1/BRCA2)
- Axillary node dissection performed; 2/15 nodes positive for adenocarcinoma (consistent
- with results of the Pathwork Tissue of Origin Test)—potentially curative surgery
